INTRODUCTION
Form 1008 is a one page document used to summarize key information utilized in the comprehensive risk assessment of the mortgage and the final underwriting decision -- such as loan purpose, property type, proposed monthly payments, and debt ratio. The Uniform Underwriting and Transmittal Summary (1008) was revised on January 1st, 2004. Lenders will be required to use the new form after April 1, 2004. These changes were made by Fannie Mae and Freddie Mac in order to comply with the USA Patriot Act and the Home Ownership and Equity Protection Act (HOEPA).

NOTE: The 1008 Transmittal Summary Software requires Adobe Reader Version 6.0 to function properly. If you are experiencing printing problems most likely Adobe Reader Version 7.0 has been installed and needs to be removed in order for the Software to work as designed. 1008 Software appears on the screen, just like the official document, but with data entry fields. Fill out the form with as much detail as you like. Data entered may be stored/saved anywhere on your hard drive, to a network shared drive, or to external diskettes as a .uts file for future editing.
