Form 1008 is a one page document used to summarize key information utilized in the comprehensive risk assessment of the mortgage and the final underwriting decision -- such as loan purpose, property type, proposed monthly payments, and debt ratio. The Uniform Underwriting and Transmittal Summary (1008) was revised on January 1st, 2004. Lenders will be required to use the new form after April 1, 2004. These changes were made by Fannie Mae and Freddie Mac in order to comply with the USA Patriot Act and the Home Ownership and Equity Protection Act (HOEPA).
